Learn About Becoming an Organ Donor Today

Understanding Organ and Tissue Donation: The Basics

Organ and tissue donation is a medical process where a person contributes one or more of their organs or tissues to help someone else who needs a transplant. When a person dies, healthy organs and tissues can be removed and transplanted into recipients who are waiting for them. According to the Organ Procurement and Transplantation Network (OPTN), over 41,000 organ transplants were performed in the United States in 2023, saving thousands of lives. However, nearly 90,000 people remain on the national transplant waiting list at any given time, and about 17 people die each day while waiting for a transplant.

The types of organs that can be donated include the heart, lungs, liver, kidneys, pancreas, and small intestine. These are called solid organs. Tissues that can be donated include corneas (which restore sight), heart valves, skin, bone, cartilage, and tendons. A single organ donor can potentially help up to eight people, while a tissue donor can help up to 75 people. This makes donation a powerful way to impact multiple lives through one decision.

Donation happens through two main pathways: after death (called deceased donation) and from living donors. Most organ donation occurs after a person has died and been declared brain dead or has experienced circulatory death. Living donation typically involves kidneys or portions of the liver, since people can live healthy lives with one kidney or with a partial liver that regenerates over time.

Understanding the basics of how donation works helps people make informed decisions about whether this is something they want to do. The process involves medical evaluation, surgical removal of organs or tissues, preservation of these materials, matching with recipients, and transplant surgery. Each step is carefully managed by medical professionals to ensure the best outcomes for both donors and recipients.

Practical Takeaway: Organ and tissue donation can save or improve the lives of multiple people. Learning about what organs and tissues can be donated, how many people are waiting, and the basic process helps you understand why this decision matters to so many families in your community.

How to Register as an Organ Donor

Registering as an organ donor is straightforward and can be done through several methods. The most common way is through your state's organ donor registry. Every state in the United States maintains its own registry where people can register their donation decision. You can typically register online through your state's health department website or the national Donate Life America website, which provides links to each state's specific registry. The process takes just a few minutes and involves providing your name, date of birth, driver's license number, and confirming which organs and tissues you wish to donate.

Another way to register is when you renew or apply for your driver's license or state ID. At the Department of Motor Vehicles (DMV), you can indicate your wish to be an organ donor. Many states use this as their primary registration method, and it's one of the most effective ways to ensure your decision is recorded. When you visit the DMV, staff members will ask if you want to register as a donor, and you can say yes or no. This registration is stored in the state's database and can be accessed by transplant coordinators when needed.

You can also register through your healthcare provider's office. Some hospitals and medical centers have registration stations, and some doctors' offices can help you register. Additionally, many organ procurement organizations (OPOs) in your region offer registration services. There are 57 OPOs across the United States, each responsible for coordinating organ donation in their geographic area. You can search for your local OPO on the OPTN website to find contact information and registration options near you.

It's important to note that registering takes very little time and costs nothing. You can change your mind about donation at any point by contacting your state's registry and requesting removal. Your decision will be documented and honored. Some people choose to register and then tell their family about their decision, which is recommended so that loved ones understand what you want.

Practical Takeaway: You have multiple options for registering as an organ donor, from online registration to doing it at the DMV. Choose the method that works best for you, complete the process in just a few minutes, and consider telling your family about your decision.

Medical Conditions and Donor Suitability

Many people worry that they won't be able to donate because of health conditions or age. The truth is that more people can donate than most realize. There is no upper age limit for organ donation—people in their 60s, 70s, 80s, and even 90s have successfully donated organs. Similarly, people with conditions like high blood pressure, diabetes, or heart disease may still be able to donate some organs or tissues. The final decision about whether organs or tissues are usable rests with medical professionals at the time of death, based on testing and evaluation.

Even if someone cannot donate organs due to certain conditions, they may still be able to donate tissues like corneas, bone, or skin. For example, a person with diabetes might not be a candidate for kidney donation but could donate corneas to restore someone's sight. Tissue donation has different medical criteria than organ donation, which means more people may have tissues that can be used for transplant.

There are very few absolute reasons someone cannot be a donor. Current guidelines state that conditions such as active cancer (in some cases), untreated HIV or hepatitis C, and severe infections may prevent donation. However, medical knowledge changes, and what was once considered a barrier to donation may now be acceptable. For instance, organs from donors with HIV or hepatitis C can now be transplanted to recipients with the same conditions, expanding the donor pool.

Age alone is not a barrier. A newborn, child, or elderly person can all be donors. The medical team will evaluate each potential donor individually. Someone who is registered as a donor should not worry that a health condition will prevent them from helping others. Registration itself doesn't commit anyone to anything—it simply expresses your wishes. Medical professionals will make the final determination about what can be used based on thorough evaluation at the time of death.

Practical Takeaway: Most people can be organ or tissue donors regardless of age or common health conditions. Don't assume you're ineligible based on a diagnosis or age. Register your wishes and let medical professionals determine what can be used to help others.

The Donation Process After Death

Understanding what happens after death during the donation process helps people feel more comfortable with their decision. When a person dies in a hospital, ICU, or emergency department, and their organs or tissues might be usable for transplant, a hospital staff member trained in donation (called a donation specialist or transplant coordinator) will contact the family. This person will explain the donation process, answer questions, and discuss the family's wishes. The approach is respectful and sensitive, recognizing that families are grieving.

If donation proceeds, the potential donor's body is taken to the operating room, similar to any surgical procedure. Surgeons remove the organs and tissues using sterile techniques, just as they would in any other surgery. The process is careful and respectful. After the organs and tissues are removed, the body is treated respectfully and can still be viewed by family members and buried or cremated according to the family's wishes. Donation does not prevent a funeral service or viewing.

Once removed, organs and tissues are preserved and transported to recipients who are waiting. The preservation process depends on the type of organ. Hearts and lungs can be preserved outside the body for only a few hours, so they must be transported quickly via helicopter or airplane. Kidneys can be preserved for up to 36 hours. Tissues like corneas, bone, and skin can be preserved for longer periods, ranging from days to years using special techniques.

The matching process involves sophisticated computer systems that consider factors like blood type, tissue type, body size, time on the waiting list, and medical urgency. Transplant coordinators use this information to find the best possible matches for each organ. A single donor's organs may be distributed to different hospitals across the country or region, with each organ going to the person best suited to receive it.

Practical Takeaway: The donation process is performed with the same medical standards and respect as any surgery. Families are kept informed and supported, and organ recipients are carefully matched. Your body remains intact enough for funeral services after donation.
