Understanding Earwax: What It Is and Why Your Ears Produce It

Earwax, also called cerumen, is a waxy substance produced by glands in the ear canal. Many people think earwax is a sign of poor hygiene, but that's not accurate. Earwax is actually a natural and important part of how your ears work. Your body produces it for good reasons.

The ear canal contains special glands that release oils and sweat, which mix together with dead skin cells to form earwax. This combination creates a protective barrier inside your ear. Earwax serves several functions: it traps dust, bacteria, and other particles before they can reach deeper into the ear; it keeps the ear canal moist and prevents dry, itchy skin; and it contains antimicrobial properties that help fight off infections.

The amount of earwax your body makes varies from person to person. Genetics play a role in how much wax you produce and what type you have. Some people naturally produce more earwax than others. Age also matters—older adults sometimes produce less earwax, which can make ears feel drier. Some medical conditions, like eczema or psoriasis, can affect earwax production too.

Normally, earwax moves out of your ear on its own. Jaw movements during chewing and talking help push old wax outward. For most people, earwax falls out naturally or washes away during bathing. Studies show that about 5 to 10 percent of children and 20 to 65 percent of older adults develop earwax buildup that causes problems. However, the majority of people never experience significant earwax issues.

Practical Takeaway: Earwax is not dirty or abnormal. It's a protective substance your body makes naturally. Understanding this helps you avoid over-cleaning your ears, which is one of the main reasons people develop earwax problems in the first place.

When Earwax Becomes a Problem: Recognizing Impaction

Earwax impaction occurs when wax builds up in the ear canal and hardens, blocking sound from reaching the eardrum. This is the most common ear problem that doctors see. People sometimes cause impaction themselves by trying to clean their ears with cotton swabs, bobby pins, or other objects. These items can push wax deeper into the ear instead of removing it.

Common symptoms of earwax impaction include conductive hearing loss (muffled hearing in one or both ears), a feeling of fullness or pressure in the affected ear, tinnitus (ringing or buzzing sounds), ear pain or discomfort, and sometimes dizziness or a cough. Not everyone with earwax buildup has symptoms—some people don't realize they have a blockage until a doctor checks their ears.

Certain situations make earwax impaction more likely to occur. People who wear hearing aids or earbuds regularly are at higher risk because these devices can block the natural outward movement of wax. Older adults with thinner ear canals or those who produce particularly dry earwax are more prone to problems. People with developmental disabilities, those who use cotton swabs frequently, and individuals with narrow or curved ear canals may also experience more impaction issues. Research indicates that about 4 million Americans seek medical treatment for earwax impaction each year.

It's important to distinguish between normal earwax and a problematic buildup. A small amount of visible wax at the ear opening is normal. However, if you notice sudden hearing changes, persistent ear discomfort, or fluid drainage from your ear, these warrant attention. Dizziness combined with ear symptoms can also indicate impaction or other ear issues that need evaluation.

Practical Takeaway: Learn to recognize the difference between normal ear maintenance and actual impaction. Sudden hearing changes or persistent discomfort are signs to stop self-cleaning attempts and seek information about professional options.

Safe Home Methods for Earwax Removal

Several home-based approaches can help with earwax removal when impaction is mild to moderate. These methods are safest when earwax is still somewhat soft and hasn't fully hardened. One widely used approach involves softening the wax first, then letting gravity and natural ear canal movements help it exit.

Oil-based solutions are commonly used for softening earwax. Mineral oil, baby oil, coconut oil, or specialized earwax softening drops can be applied to the ear canal. To use this method: lie on your side with the affected ear facing upward, place 2-3 drops of the chosen oil into the ear opening, keep your head tilted for 5-10 minutes to allow the oil to coat the wax, then lie on your other side to let it drain. Many people do this for several days before attempting removal. Some find that doing this before bed allows the oil to work overnight.

Water-based methods can also help. Warm (not hot) water or saline solution, either plain or mixed with hydrogen peroxide at lower concentrations, can soften wax and promote drainage. Ear irrigation kits are available without a prescription at most pharmacies. These typically include a small bulb or squeeze bottle and instructions for safe use. The key is using body-temperature liquid and allowing gravity to work—never force liquid into the ear with excessive pressure.

Over-the-counter earwax removal products containing carbamide peroxide or hydrogen peroxide are designed to bubble and loosen impacted wax. These products come as drops or sprays and typically require application several times daily for 3-7 days. Follow package instructions carefully, as overuse can irritate the ear canal. Some people combine softening drops with gentle manual removal techniques, though this should be done carefully and only when wax is soft enough to come away easily.

Important safety guidelines include: never insert objects deeper than the outer ear canal, never use excessive force, stop immediately if you experience pain or bleeding, and avoid any method if you suspect your eardrum might be damaged or perforated. People with ear tubes, a history of eardrum perforation, or ear drainage should not attempt home removal without medical consultation.

Practical Takeaway: Home softening methods work best for mild buildup and require patience—typically 3-7 days of treatment. Gentleness and avoiding objects that penetrate deeply are essential to preventing damage.

Medical Earwax Removal Procedures and When to Seek Them

When home methods don't work or earwax impaction causes significant symptoms, medical professionals can remove wax safely and effectively. Primary care doctors, ear, nose, and throat specialists (called ENTs or otolaryngologists), and some nurse practitioners and physician assistants perform earwax removal. Audiologists who fit hearing aids may also provide this service.

The most common in-office procedure is cerumen removal with instruments called curettes or loops. The doctor uses an otoscope to see inside the ear canal, then carefully uses a small curved instrument to scoop out hardened wax. This procedure is quick, usually taking just a few minutes, and patients can return to normal activities immediately. Some people experience mild discomfort during the procedure, but it's generally not painful when done by experienced professionals.

Suction removal is another method. The doctor inserts a small tube connected to controlled suction, which pulls the wax out. This is often preferred for people who are sensitive to instruments in their ears. Suction works well for both hard and softer wax buildup.

Irrigation, performed in a medical setting with proper equipment and technique, can also remove earwax. Doctors use specialized irrigation devices that direct warm water or saline at a controlled pressure and angle into the ear canal. This is safer than home irrigation because medical-grade equipment prevents excessive pressure buildup. Irrigation typically works best when wax has been softened with drops beforehand.

Seek professional removal if you've tried home methods for one to two weeks without improvement, if you experience significant hearing loss or discomfort, if you have drainage from your ear, if you have a history of eardrum problems, if you wear hearing aids that are blocked, or if you have a developmental disability that makes self-care difficult. Medical removal is also recommended for very hard or impacted wax, wax that keeps returning quickly, or if home attempts cause pain or bleeding.

The cost of professional earwax removal typically ranges from $75 to $200 depending on your location and whether it's performed in a doctor
