Learn About WIC Programs and How They Work

What Is the WIC Program and Who It Serves

The Women, Infants, and Children (WIC) program is a federal nutrition program run by the U.S. Department of Agriculture. It has been operating since 1972 and serves millions of people each month. As of 2023, WIC provided nutrition support to approximately 6.2 million participants across the United States, making it one of the largest nutrition assistance programs in the country.

WIC is designed specifically to help pregnant women, postpartum women, breastfeeding women, infants, and children up to age 5 who meet certain income and nutrition risk criteria. The program focuses on these groups because research shows that nutrition during pregnancy and early childhood has lasting effects on health, development, and learning. According to the USDA, studies have shown that WIC participation is associated with improved birth outcomes, better infant health, and improved growth and development in young children.

The program operates in all 50 states, Washington D.C., the U.S. Virgin Islands, and Guam. Each state runs its own WIC program but follows federal guidelines. This means the basic structure is similar everywhere, but some details about food packages and services may differ by state. For example, California's WIC program serves over 1 million people monthly, while Wyoming serves around 7,000 participants monthly, reflecting population differences.

WIC differs from other nutrition programs like SNAP (food stamps) in important ways. While SNAP is a general food assistance program for low-income households of any size, WIC is specifically targeted to mothers and young children. WIC also provides nutrition education, breastfeeding support, and referrals to healthcare and other services—not just food benefits. The program recognizes that feeding a family involves more than just money for groceries; it also involves education and connection to other resources.

Takeaway: WIC is a longstanding federal program created specifically to support the nutrition and health of pregnant women, new mothers, and young children through food benefits combined with education and health services.

How WIC Food Benefits Work

WIC provides participants with a monthly food package that includes specific nutritious foods. The program doesn't provide cash—instead, it works through an electronic benefit card (similar to a debit card) that can only be used to purchase approved WIC foods at authorized grocery stores and farmers markets. This approach ensures that the nutrition benefits go toward healthy foods that support the development and health of mothers and young children.

The foods included in WIC packages are based on research about nutrition needs during pregnancy, infancy, and early childhood. Typical WIC foods include milk, cheese, eggs, beans, peanut butter, whole grain bread, cereal, fruits, vegetables, and infant formula. The specific foods and amounts vary based on the participant's age and category. For example, a pregnant woman's package differs from an infant's package. A breastfeeding mother might receive different items than a woman who is not breastfeeding, since breastfeeding women have higher calorie needs.

Here's how the process works in practice: A participant receives a WIC card with their monthly food benefits loaded on it. They go to a WIC-authorized store and select only the foods listed on their WIC approval list. The cashier scans the card, and the approved foods are paid for through the WIC program. Foods not on the WIC list—such as candy, soda, meat, seafood, or pre-made meals—cannot be purchased with WIC benefits. This ensures that the program's resources support specific nutrition goals.

The food packages are designed to provide about 50-60% of daily nutritional needs across several key categories: milk and dairy products, proteins, grains, fruits and vegetables, and infant formula when needed. This means that WIC supports nutrition but doesn't cover all food costs. Families typically use other resources—such as their own money, SNAP benefits, or WIC combined with other programs—to meet their complete food needs.

Most states allow participants to use WIC benefits at multiple types of authorized retailers, including supermarkets, grocery stores, and farmers markets. According to USDA data from 2022, over 40,000 authorized retailers across the country accept WIC benefits. Some farmers markets also accept WIC benefits, which means participants can use them to purchase fresh produce directly from farmers. This encourages healthy eating habits and supports local agriculture.

Takeaway: WIC provides monthly food benefits on an electronic card that can only be used to purchase specific nutritious foods at authorized stores, helping ensure resources support the health needs of mothers and young children.

Nutrition Education and Health Services Included in WIC

A key feature that sets WIC apart from other food assistance programs is the inclusion of nutrition education and health services. While the food benefits are important, the program also recognizes that families benefit from learning about nutrition, feeding practices, and how to access other health resources. Each state WIC program offers nutrition education, though the specific content and format can vary.

Nutrition education in WIC covers topics relevant to mothers and young children. Common topics include breastfeeding support and information, how to introduce solid foods to infants, choosing healthy foods on a budget, preparing nutritious meals for families, and how different foods support child development. Many WIC programs offer this education through individual counseling sessions, group classes, or both. For example, some programs offer classes on meal planning or cooking demonstrations featuring WIC-approved foods.

Breastfeeding support is a significant component of WIC. The program employs breastfeeding peer counselors and lactation consultants in many states to provide one-on-one support to pregnant women and new mothers. According to USDA data, about 52% of WIC participants who are postpartum women are breastfeeding, and WIC aims to increase this rate through education and support. Breastfeeding counselors can discuss benefits of breastfeeding, help with common challenges, and connect mothers to additional resources.

WIC programs also provide referrals to other health and social services. This might include connecting participants to prenatal care, pediatric healthcare, substance abuse services, domestic violence resources, mental health services, or other community programs. WIC staff are trained to recognize when participants may need additional support and can provide information about where to find those services. For example, if a pregnant woman mentions she's struggling with depression, the WIC program can provide information about mental health resources in her area.

Some WIC programs have expanded to include services like text message reminders about appointments and nutrition tips, online nutrition education modules, or virtual counseling sessions. These additions help reach participants who may have challenges attending in-person appointments due to work, transportation, or childcare responsibilities. A 2021 study found that WIC participants who received additional nutrition education showed improvements in dietary quality compared to those who didn't receive the extra education.

Takeaway: WIC includes nutrition education and connection to health services as core components, recognizing that supporting families involves more than just providing food.

Income Guidelines and Nutrition Risk Factors

WIC has two main requirements that must typically be met: income guidelines and nutrition risk. Understanding these factors provides insight into how the program determines who can participate. However, it's important to note that specific rules and limits are set by each state, so the exact details vary by location.

Income guidelines for WIC are generally set at or below 185% of the federal poverty line, though some states set them lower. For 2024, the federal poverty line for a family of three is approximately $23,050 annually. This means a family of three with income up to about $42,642 might fall within WIC income guidelines in states using the 185% standard. However, this is just a general example—each state can set different limits within federal guidelines, and the limit may be adjusted annually. Participants should look up their specific state's current income limit, as it can change year to year based on federal poverty guidelines.

Beyond income, WIC also requires that participants have a nutrition risk factor. Nutrition risk means there is evidence of a health or dietary problem related to nutrition. A healthcare provider (such as a doctor or nutritionist) typically makes this determination. Examples of nutrition risks include:

• Anemia (low iron in the blood)
• Inadequate weight gain during pregnancy
• Underweight infants or children
• History of poor diet
• Certain chronic health conditions that affect nutrition
• Overweight or obesity
