Learn How PET Scans Work and What They Do

What Is a PET Scan and How Does It Differ From Other Medical Imaging?

A PET scan stands for Positron Emission Tomography scan. It is a type of imaging test that allows doctors to see how your body is working at the cellular level, rather than just showing the structure of your organs and tissues. Unlike CT scans or X-rays that primarily show the shape and size of organs, PET scans reveal metabolic activity — meaning they show which parts of your body are using the most energy and glucose.

The PET scan works by detecting radiation given off by a special tracer substance. This tracer is injected into your bloodstream before the scan. The tracer contains a radioactive element that emits positrons, which are tiny particles of energy. When positrons meet electrons in your body, they create small bursts of energy that the PET scanner detects. These energy bursts are converted into images that show areas of high and low activity in your body.

This is fundamentally different from other imaging methods. An MRI scan uses magnetic fields to create detailed pictures of soft tissues. A CT scan takes multiple X-ray images and combines them into cross-sections of your body. An ultrasound uses sound waves to create images. But a PET scan shows functional information — it tells doctors how actively your cells are behaving, not just their appearance. This makes PET scans particularly valuable for detecting cancer, heart disease, and neurological disorders in early stages.

Many times, doctors use PET scans combined with CT scans in a test called a PET-CT scan. This combination provides both structural and functional information in one exam. The CT portion shows exactly where in your body something is located, while the PET portion shows which areas are most metabolically active. This combination helps doctors pinpoint problems with greater accuracy than either scan alone could provide.

Practical Takeaway: Understanding that PET scans show how your body is working, not just its structure, helps explain why doctors might order this test when other imaging methods have not provided enough information about a medical concern.

The Science Behind How PET Scanners Detect Activity in Your Body

The technology behind PET scanning relies on nuclear physics principles that might seem complex, but the basic concept is straightforward. Before your scan, you receive an injection of a radioactive tracer. The most common tracer used in PET scans is called FDG, which stands for fluorodeoxyglucose. FDG is a form of glucose, the sugar your cells use for energy, with a radioactive fluorine atom attached to it. Your body treats this radioactive glucose the same way it treats regular glucose, so it accumulates in areas where cells are most active and consuming the most energy.

Once the tracer enters your bloodstream, it travels throughout your body. Cancer cells, areas of infection, and inflamed tissues typically consume more glucose than normal healthy cells because they require more energy. The radioactive tracer therefore collects in higher concentrations in these problem areas. Within about an hour of injection, the tracer has distributed throughout your body and concentrated in the areas where it will be detected.

The PET scanner itself is a large, doughnut-shaped machine. You lie on a table that slowly moves through the center of this doughnut. Inside the scanner are thousands of tiny radiation detectors arranged in rings. As the radioactive tracer in your body emits positrons, these particles almost immediately collide with electrons. When a positron and electron meet, they annihilate each other and release energy in the form of gamma rays that shoot off in opposite directions. The detectors pick up these gamma rays and record their location.

A computer processes all this detection data and creates three-dimensional images of your body. Areas with high tracer concentration appear as bright spots or areas of color, depending on how the images are displayed. Areas with low tracer concentration appear darker. Different colors may represent different levels of activity, with red or yellow typically indicating the highest activity and blue or black indicating the lowest. The entire scanning process typically takes 20 to 40 minutes, depending on which parts of your body are being scanned.

Practical Takeaway: Knowing that PET scans work by tracking where your body uses the most energy helps you understand why they are so good at finding cancer and other diseases that make cells behave abnormally.

Common Medical Uses and What Conditions PET Scans Can Help Diagnose

PET scans have become valuable tools for diagnosing and monitoring several serious medical conditions. The most common use is detecting and staging cancer. Approximately 90 percent of PET scans performed in the United States are used for cancer-related purposes. This includes finding tumors that may not show up on other imaging tests, determining whether cancer has spread to other parts of the body, and monitoring whether cancer treatment is working. PET scans can detect many types of cancer, including lung cancer, breast cancer, colorectal cancer, melanoma, and lymphoma.

Beyond cancer detection, PET scans are increasingly used to evaluate heart disease. A cardiac PET scan can show which areas of your heart muscle are receiving adequate blood flow and which areas may have reduced blood flow due to blocked arteries. This information helps cardiologists determine if you are at risk for a heart attack and whether interventions such as medication or angioplasty might help. Doctors can also use PET scans to assess how well your heart is functioning after a heart attack or heart surgery.

Neurological conditions represent another major category of PET scan use. Doctors order PET scans to help diagnose and monitor Alzheimer's disease, Parkinson's disease, epilepsy, and other brain disorders. In Alzheimer's disease, PET scans can show patterns of reduced glucose metabolism in specific brain regions years before a person develops noticeable symptoms. This early detection capability may eventually help doctors start treatment sooner, potentially slowing the progression of cognitive decline.

PET scans can also help identify areas of infection or inflammation. In patients with fever of unknown origin, a PET scan may pinpoint where in the body an infection is occurring. Patients with chronic inflammatory conditions can use PET scans to see which areas of their body have the most inflammation. The scan results help doctors decide on the best treatment approach and monitor whether treatment is reducing inflammation.

Additional uses include detecting bone metastases (cancer spread to bones), evaluating thyroid nodules, and assessing areas of scar tissue in the heart. In research settings, PET scans are helping scientists better understand how the brain functions and how various diseases develop.

Practical Takeaway: PET scans serve many different medical purposes, but they are most commonly used when doctors suspect cancer or need to understand how well your heart or brain is functioning at a cellular level.

What to Expect Before, During, and After a PET Scan Procedure

Preparation for a PET scan begins several days before your appointment. You will receive written instructions from your imaging facility explaining what to do. One important preparation step involves limiting your physical activity for 24 hours before the scan. Strenuous exercise causes your muscles to consume more glucose, which can interfere with the scan results by creating misleading areas of high activity. You should continue your normal daily activities, but avoid intense workouts or sports.

Dietary restrictions typically apply before a PET scan as well. You will usually be asked to fast, meaning you should not eat or drink anything except water, for several hours before the procedure. This fasting period allows the radioactive tracer to concentrate properly in disease areas rather than being diluted by glucose from food you recently consumed. If you take medications, ask your doctor which ones you should or should not take on the morning of your scan. Some medications can interfere with scan results.

You should wear comfortable, loose-fitting clothing that does not contain metal zippers, snaps, or buckles. Metal can interfere with some scanning equipment. Arrive at the imaging facility about 15 minutes before your appointment time. A technician will explain the procedure, answer your questions, and ask you to sign consent forms. You will also be asked about any medical implants such as pacemakers or metal joints, since these may affect how the scan is performed.

During the actual procedure, a nurse or technician will insert an intravenous line, usually in your arm. Through this IV, they will inject the radioactive tracer. After the injection, you will typically wait in a quiet, dim room for about an hour while the tracer circulates through your body and collects in areas of disease or high activity. During this waiting period,
